Flea/Tick and Heartworm Prevention require:

-Up to Date Exam (annually)

-Heartworm Test (annually)

*If your pet is on heartworm prevention year round  (ALL 12 months), a heartworm test is only required once every two years

-Up to Date Exam (annually)

-Bloodwork as Needed (determined by the veterinarian)

Online Pharmacy Requests require:

-Up to Date Exam (annually)

-Bloodwork as Needed (determined by the veterinarian)

-Online Pharmacy Waiver (only needs signed once)
